You will be asked to set up a username and password and then it will it guide you through the paperwork and you can access my schedule. You will complete demographic form and then a biophsyco-social form. When you see my schedule, if times are listed on the calendar then this is what is available. If blank, the day is full. Please keep a copy of your username and password. If you loose your password you are welcome to contact me and I can set up a temporary password for you.
Once you have completed the paperwork and scheduled an appointment, you will be notified that you have an appointment the day before your appointment time by text, phone or email depending on which information you provide in the paperwork.
