Please click the link below and you will be able to access my schedule.  For new patients, you will need to create a username and password and then complete paperwork online.  For returning clients who were seen prior to September 2014, you will need to register as a new client.  For returning clients who have been seen after September 2014, a username and password should have been established already for you.  If you don't know or forgot your username and password, call or text me and I can get this to you. 435-590-4411
